Rehabilitation Services 
 
St. Jude Care Center    Each of our residents comes to us with very different and specific rehab needs. Our entire rehabilitation team is committed to create individualized therapy programs that directly answer those specific needs. We encourage the resident to achieve their maximum outcomes of independence and ability. Families are also encouraged to participate in therapy sessions.

Nursing Services 
 
   The nursing staff seeks to maintain a calming environment where our residents can rest and recover and their families feel informed. The following skilled nursing services are available at St. Jude Care Center:
  • Medical Director coordinates and improves medical services
  • Director of Nursing oversees all aspects of healthcare within the facility
  • Individualized resident care plan treatments
  • Dementia care

Restorative Nursing Program 
 
   We seek to maximize the quality of life for our long-term care population and offer them specialized restorative services. This service is included in the daily room and board fee. The therapy team and nursing staff develop individualized treatment plans for resident care in the areas of:
  • Activities of daily living
  • Ambulation
  • Bowel and bladder training
  • Contracture management
  • Weekly and monthly height and weight monitoring
Physical Therapy 
 
   We offer PT six days a week for your convenience and faster recovery. Physical therapy works toward specific goals of achieving the residentsí highest functional level of independence. We focus on identification, prevention and rehabilitation of individuals with physical impairment and pain. Following evaluation, the therapists develop an individualized treatment program.

Areas of treatment focus may include:
  • Increase strength and mobility in the body and lower extremities
  • Increase motor control for a resident who has suffered a stroke
  • Improve coordination, balance and the ability to walk following a fracture
  • Improve overall strength conditioning, range of motion and endurance
  • Pain management
Occupational Therapy 
 
   Occupational therapists use task-directed activities in the treatment of our residents whose functional abilities, daily activities and independence have been impaired by illness, injury or aging. Occupational therapy assist our residents in adapting to their social and physical environment by mastering tasks essential for daily living. Our therapists may work with residents who have debilitating arthritic conditions or difficulty bathing, dressing and other activities of daily living. They assist residents with impaired vision and/or hearing to function well in their environment.

Areas of treatment focus may include:
  • Increase strength and mobility in the body and lower extremities
  • Functional independence: eating, bathing, toileting, homemaking, cooking, etc.
  • Maintenance of wellness: dressing and grooming
Speech Therapy 
 
   Speech therapists provide a range of services to assess, educate and train the resident in communication, swallowing and cognition. Treatment programs are designed to help residents affected by stroke, brain injury, dementia, and other neurological disorders. Our therapists also work with residents suffering from hearing impairment, respiratory illnesses, and complex medical conditions.

Areas of treatment focus may include:
  • Increase strength and mobility in the body and lower extremities
  • Improving speech and voice clarity
  • Increasing comprehension and verbal expression
  • Improving chewing and swallowing ability
  • Residents admitted to the facility that receive nutrition partially or exclusively via a gastrostomy tube are evaluated for potential return to solid foods as appropriate.
Activities 
 
   A positive and uplifting atmosphere is vital to the success of healing, recovery and rehabilitation. St. Jude Care Center employs outgoing and friendly professional staff to create social programs and activities to fit the resident needs. We provide a monthly activity calendar to each resident to keep them informed of upcoming events. Friends and families are always welcome to visit and attend our special events.
 
Social Services 
 
   Social services provide essential information, manage requests and concerns, coordinate appointments, maintain contact and open communication between the facility, families and the residents, help in care planning and discharge planning for each resident. We stand up for resident's rights and strive to make St. Jude Care Center their home.
